All new clients are asked to complete a health questionnaire and a consent form, this allows Hypopressives North Wales to collect information about you to generate a client record. You will be asked for the following basic information:

      • Date of birth
      • Postcode
      • Emergency contact name and telephone number

Session notes and forms you have completed and are all stored securely.

Retention

Your records are kept securely in accordance with Data Retention Policy for 8 years. After this time your records will be securely destroyed.

If you have otherwise booked a ticket or associated products and services with us or contacted us with a question or comment, we shall retain your personal data for 6 months following such contact to respond to any further queries you might have.
